We honor the following  C4C Community Health workers.  Our five honored members have unfortunately passed away over the past decade & two.  They each served our diverse Communities with enthusiasm as they motivated each and all whom crossed their path. As they each served C4C, they planted within the agency seeds of vision. Each motivated and inspired us to continue in unwavering service to our families as a whole, and our community at large.  Their spirit lives on within the heart of C4C.
